Submit A Privacy Request

Use this form to ask us about the personal information we hold about you.

You can use this form to ask us to confirm what personal information we have about you, to give you access to it or a portable copy of it, to correct something that is wrong, or to delete it. You can also use it to appeal a decision we made on an earlier request.

There is no charge to submit a request, and using this form will not affect your coverage, your premium, or how we work with you.

Other ways to reach us

If you would rather not use the form, you can send the same request by email to khowton@infinitygroupins.com or call us at (972) 298-3800. All three methods reach the same place and are handled the same way.

What happens next

  1. We confirm we received it. You will get an acknowledgment at the email address you give us.
  2. We may need to verify who you are. Before we hand over or change personal information, we have to be reasonably sure you are who you say you are. We may ask you for a detail or two that lets us match your request to our records. Anything you send us for verification is used only for that, and nothing else.
  3. We respond within 45 days. If a request is complex or we are handling several from you at once, we may need up to another 45 days, and we will tell you before we take it.
  4. If we say no, we tell you why. And you can appeal.

Why we sometimes have to say no

We are a licensed insurance agency, so a good deal of what we hold is subject to insurance recordkeeping rules and to federal financial privacy law. That means we may not be able to delete or change information that:

  • We are required to retain under insurance, tax, or other legal recordkeeping obligations;
  • We need in order to service a policy that is currently in force, or an open claim;
  • Is held by an insurance company rather than by us — in which case we will point you to the right carrier;
  • Is exempt from state privacy law because it falls under the Gramm-Leach-Bliley Act.

If any of this applies, we will say so plainly and explain which part of your request we can act on.

Appeals

If we decline your request in whole or in part, you may appeal within a reasonable time by replying to our response, or by contacting us again through this form and selecting “Appeal a decision on a previous request.” We will respond in writing within 60 days and explain our reasoning. If we deny the appeal, we will give you a way to submit a complaint to the Texas Attorney General.

Submitting on someone else’s behalf

You may use an authorized agent to submit a request for you. We may ask the agent for proof that you authorized them, and we may also contact you directly to confirm. If you are submitting a request for someone else, please say so on the form.

If you are an Infinity Group Insurance customer, a separate privacy notice required by federal and Texas insurance law governs the nonpublic personal information you give us in connection with your policies. Ask us and we will send you a copy.
